Primary Care Patients

Patients for whom we provide all care, including check ups, immunizations, etc.

I recognize that changing physicians may be stressful. As I was unable to procure a primary care physician to assume my practice, I advise parents, in order to ensure continuity of care, to find a new family physician or primary care pediatrician through personal enquiries or by contacting Health Care Connect at 1-800-445-1822 or  www.ontario.ca/healthcareconnect  in order to find a new physician.

All patient medical records will be placed in storage and, upon request, a copy will be transferred to your new physician for a small administrative fee.– See Chart Transfer Requests here.

I will certainly miss my patients and parents who felt like family members. It was a privilege and an honour to provide your Pediatric care.
