Chart Transfer Requests for Primary Care Patients Only

All patient medical records have been placed in storage with the medical records storage company Docudavit Solutions. You may contact them at 1-888-781-9083 to request a chart transfer. Please note, in some cases, there may be a combination of both a paper chart and electronic record chart, thus ask Docudavit to check for both.There will be an administration fee as per Ontario Medical Association guidelines based on the complexity of the chart.
