Consult Patients With a Current Family Physician

Consult patients are those that were initially referred to me by their family physician for a specific medical problem.

I understand that changing physicians may be stressful. I was unable to arrange transfer of my consult patients to another mental health specialist physician. Patient care, and renewal of prescriptions will revert to your family physician as of July 28, 2023. Unfortunately, the other physicians at Capital Kidz physician group will be unable to provide my former patients’ prescription renewals or ongoing care.

Your family physician already has a copy of all clinic visits with Dr. Bialik. Therefore, we will not provide chart transfer notes to a new mental health physician as your family physician can provide a more thorough note.

Your family physician can refer your child to another paediatric mental health expert, or use the E consult service from CHEO psychiatry for advice as needed.
