Consult Patients Without a Family Physician

Consult patients are those that were initially referred to me by their family physician for a specific medical problem.

I understand that changing physicians may be stressful. I was unable to arrange transfer of my consult patients to another mental health specialist physician.

Unfortunately, the other physicians at Capital Kidz physician group will be unable to provide my former patients’ prescription renewals or ongoing care.

All patients without a family physician will have to have prescriptions renewed, and a new referral to another pediatrician obtained, at the local emergency room or walk-in clinic.

I strongly encourage you to attempt to find a family physician by contacting Health Care Connect at 1-800-445-1822 or www.ontario.ca/healthcareconnect.
